The fix for most people is to install Oblivion Stutter Remover and configure it with bReplaceHeap=1 and iHeapAlgorithm=5.
There is another thread on the Technical Support forum that discusses this issue at length. Cause 1 Other programs disrupting disk operation, or you have old save games and settings in the (My Documents/My Games/Oblivion). Oblivion crashes on startup I don't even get to the main menu.
